LIABILITY WAIVER AND RELEASE OF CLAIMS

Renzo Gracie Lone Star Team – Pflugerville

I, the undersigned participant (or parent/legal guardian if the participant is a minor), understand that Brazilian Jiu-Jitsu, wrestling, judo, mixed martial arts, fitness training, and related activities involve inherent risks, including but not limited to:

• Slips, falls, and collisions

• Joint sprains and strains

• Broken bones

• Concussions

• Cuts and bruises

• Serious bodily injury, permanent disability, or death

I voluntarily choose to participate in training and activities at Renzo Gracie Lone Star Team – Pflugerville.

1. ASSUMPTION OF RISK

I knowingly and freely assume all risks, both known and unknown, even if arising from the negligence of the releases or others.

2. RELEASE AND WAIVER OF LIABILITY

I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, and forever discharge:

• Hansell & Garza, LLC

• Renzo Gracie Lone Star Team

• Renzo Gracie Lone Star Team – Pflugerville

• All owners, officers, managers, employees, instructors, volunteers, contractors, members, and affiliates

from any and all liability, claims, demands, losses, or damages arising out of or related to any injury, disability, death, or property damage that may occur while participating in any activities at the facility or during any affiliated event.

3. COVENANT NOT TO SUE

I agree that I will not file any lawsuit, claim, or legal action against any of the parties listed above for any injury, damage, or loss arising from my participation, except where prohibited by Texas law.

4. MEDICAL CONSENT

I certify that I am physically able to participate. I consent to emergency medical treatment if necessary and understand that I am responsible for any related medical costs.

5. RULES AND SAFETY

I agree to:

• Follow all gym rules and instructor directions

• Tap early and release submissions immediately when a partner taps

• Maintain personal hygiene and safe training practices

• Inform staff of any injuries or medical conditions

I understand that failure to follow safety rules may result in removal from training without refund.

6. EQUIPMENT AND PROPERTY

I understand that I am responsible for my personal property and that the gym is not liable for lost, stolen, or damaged items.

7. MINOR PARTICIPANTS

If signing for a minor, I certify that I am the parent or legal guardian and agree to all terms on their behalf.

8. PHOTO AND VIDEO RELEASE

I grant permission for the use of photos or videos of me/my child for promotional purposes without compensation.

9. SEVERABILITY

If any portion of this agreement is held invalid, the remaining portions shall remain in full force and effect.

10. GOVERNING LAW

This agreement shall be governed by the laws of the State of Texas.

11. ACKNOWLEDGEMENT

I have read this waiver, fully understand its terms, and sign it freely and voluntarily. I understand that I am giving up substantial legal rights, including the right to sue.
